Location: Lexington, KY

End Date: 10-26-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

• Provide comprehensive physical therapy evaluations and treatments to home health patients
• Develop and implement personalized care plans for recovery and independence
• Perform start of care, recertifications, evaluations, and daily visits
• Collaborate with clinical managers and educators for guidance and support
• Document patient progress using Homecare Homebase (HCHB) charting system
• Educate patients and families on exercises and home safety

Qualifications

• Active Physical Therapist license in Kentucky (or compact eligible)
• Minimum 1 year of experience in home health setting
• BLS certification from the American Heart Association required
• Must have experience with Home Care Home Base (HCHB) and home health
• Ability to work independently and manage a caseload of 6.5 points per day

Knowledge & Skills

• Expertise in home health therapy including SOC, ROC, recerts, and evaluations
• Proficiency with Homecare Homebase (HCHB) electronic medical records
• Strong clinical reasoning and patient assessment skills
• Excellent communication and patient education abilities
• Knowledge of common diagnoses: CHF, Orthopedic conditions, Pneumonia
